Job Title: RN – Cardiac Stepdown/PCU
Location: Tower 2
Start Date: 6/30 (Start dates are non-negotiable)

Unit Overview

• Unit Type: Cardiac Stepdown/PCU (Progressive Care Unit)
• Bed Count: 36 beds
• Patient Population: Primary focus is cardiac patients. May include mixed Medical-Surgical and PCU patients.
• Common Diagnoses:
  • CHF, NSTEMI, MI, heart block, pacemaker, TAVR, CABG (Day 2), post-cath recovery after sheath removal, Watchman procedure, bradycardia, VT
  • General PCU overflow: respiratory failure, hypotension, anemia, sepsis, post-surgical patients

Staffing and Ratios

• Nurse-to-Patient Ratios:
  • Day Shift: 1:4
  • Night Shift: 1:5
  • MS patients: 1:5 ratio
• Charge Nurse: Present on each shift, typically does not take a patient assignment (may assist minimally if needed)
• Patient Care Assistants (PCAs): Ratio of 8–12 patients per PCA (may vary)
• Telemetry:
  • Unit is telemetry monitored
  • RNs must be able to interpret their own strips
  • Monitor tech is available for support
• RT Equipment: Includes BiPAP, High Flow, and stable tracheostomy care. RNs are expected to troubleshoot as needed.

Certifications Required

• BLS
• ACLS
• NIHSS

Clinical Requirements

• Experience:
  • No first-time travelers
  • Prior experience in PCU, Stepdown, or Telemetry units required
  • Prefer experience with TAVR and CABG patients (typically assigned to core staff)
• Titrating Drips (Required):
  • Amiodarone
  • Cardizem
  • Dobutamine
  • Dopamine
  • Heparin
  • Initiate Nitroglycerin
  • Insulin

Scheduling

• Shifts: 6:45 AM – 7:15 PM / 6:45 PM – 7:15 AM
• Self-Scheduling: Charge nurse creates schedule balancing unit needs; traveler requests are considered
• Weekend Requirement: 4 weekend shifts per month
  • Days: Saturday & Sunday
  • Nights: Friday, Saturday, Sunday
• Holiday Requirement: Must be available for 3 out of 5 major holidays
• Low Census: May result in full or partial day standby

Floating Requirements

• May float to ICU (within skill scope) and Medical-Surgical units

Dress Code

• Scrubs: Navy Blue (scrubs not provided)

Orientation

• 1 day of New Employee Orientation (NEO)
• 1 day with Clinical Educator
• 1 unit-specific floor orientation shift
